The Clinical Support Coordinator and LPN serves as both a clinical and administrative resource within the outpatient clinic. This position is responsible for coordinating patient access and clinic operations while performing nursing functions within the scope of Licensed Practical Nurse licensure. The role provides frontline patient support, scheduling and registration services, medication and treatment coordination, and assists providers in delivering safe, effective patient care. Duties are performed in accordance with the Wisconsin Nurse Practice Act, applicable regulatory requirements, and Rogers policies and procedures.
Job Duties & Responsibilities:
Patient Access and Clinic Operations
Serve as the primary point of contact for patients, families, and visitors. Answer telephones and provide information regarding Rogers programs and services. Register, schedule, and check patients in and out of appointments. Verify patient demographics, insurance coverage, guarantor information, and required consents. Coordinate referrals, patient transfers, and follow up appointments. Maintain clinic supplies and coordinate ordering as necessary. Maintain medical records and ensure accurate placement of documentation within the electronic health record. Complete operational reports, metrics, and assigned administrative projects. Respond to crisis intervention and emergencies.
Clinical Support Responsibilities
Collect and document patient information, including admission and discharge related information. Observe and report changes in patient condition and communicate concerns to the treatment team. Participate in patient assessments, treatment planning, and care coordination under the direction of a Registered Nurse, physician, or other authorized provider. Obtain and process physician orders and medication reconciliation activities as permitted by licensure and policy. Administer medications and monitor for therapeutic and adverse effects consistent with state regulations and organizational policy. Educate patients regarding medications, treatments, and clinic processes. Facilitate communication between patients, providers, pharmacies, insurers, and external agencies. Maintain patient confidentiality and advocate for patient rights. Assist in responding to patient concerns and escalating urgent clinical issues appropriately.

Demonstrate acceptance and training of student interns in the department, as directed.

Education/Training Requirements:
- Current Licensed Practical Nurse license in good standing required.
- High school diploma required.
- Outpatient, behavioral health, or medical office experience preferred.
- Electronic health record experience preferred.
- CPR certification required.
- Crisis prevention/intervention training required.
