Per Diem - Nurse Practitioner - Senior Community Care - Roanoke

• $3,500 Sign-On Bonus for external candidates.
• Per diem opportunity within the Senior Community Care (SCC) program serving high-risk and medically complex members.
• Coverage need is primarily afternoons and weekends.
• Expected commitment is approximately 24 hours per month.
• Provide care across home, nursing home, assisted living, and senior housing environments.
• Conduct comprehensive assessments, medication reviews, point-of-care testing, and advanced care planning discussions.
• Collaborate closely with an interdisciplinary care team to support members through rehabilitation and ongoing care needs.

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license (or ability to obtain before start date) without disciplinary actions or practice limitations.
• Active, unrestricted driver's license and reliable transportation.
• Current DEA licensure/prescriptive authority (or ability to obtain post-hire), and ability to establish a collaborative practice agreement if required by state regulations.

Compensation for this specialty generally ranges from $52.64 - $78.85 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
