Salary: $66,300-$70,000/yr

Position Summary: The Therapeutic Case Planner provides comprehensive, trauma-informed, and strengths-based services to parents and families affected by intimate partner violence (IPV). The position focuses on identifying and addressing barriers to children's health, safety, and well-being while supporting families in achieving stability, strengthening relationships, and reducing the likelihood of future involvement with the child welfare system. The Therapeutic Case Planner is responsible for conducting safety-focused assessments of children and families, developing individualized intervention and service plans, providing supportive counseling and advocacy, and helping families identify and implement sustainable solutions. Through coordinated case management and collaboration with community partners, the Therapeutic Case Planner supports parents in establishing safe, stable, and nurturing environments for their children. The Therapeutic Case Planner reports directly to the Program Supervisor.

Children's Aid annually serves children and their families at over 40 sites in New York, as well as through three national program models. Children's Aid is concentrating its efforts on creating cradle-through-college pathways in high-need neighborhoods, with a focus on children's health, family stability, education, and social and emotional development.

Essential Duties and Responsibilities:

* Understand, promote, and demonstrate the organization's Mission, Vision, and Core Values while adhering to all organizational policies, procedures and professional standards.
* Maintain a caseload of approximately 10-11 families and complete a minimum of two face-to-face contacts with each family per month.
* Conduct joint home visits with the Administration for Children's Services (ACS) and participate in establishing case disposition by explaining program requirements and benefits of program.
Job Description
* Administer the ACS Domestic Violence Screening Tool and complete all required program intake and assessment documentation within required timeframes.
* Utilize appropriate screening and assessment tools to evaluate the needs, strengths, risks and safety of children and family members.
* Provide crisis intervention, supportive counseling, psychoeducation, safety planning, ongoing safety assessments and reassessments as needed.
* Provide advocacy and support to families navigating child welfare, public benefits, law enforcement, legal and family court systems.
* Assist families in accessing and securing essential services via advocacy and referrals, including safe and stable housing, public benefits, emergency financial assistance, health care, mental health services, substance use services, educational resources, legal advocacy and representation, parenting support and other case management resources.
* Develop and maintain individualized Family Assessments and Service Plans that reflect family strengths, identified needs, safety concerns, and measurable goals.
* Maintain accurate, timely, and up-to-date case records, progress notes, contact documentation and other required records, including entering case information into the PROMIS and Connections database systems.
* Accompany and support clients at appointments and other activities necessary to access services and resources.
* Establish and maintain collaborative relationships with community-based providers and make appropriate referrals to support continuity and coordination of care.
* Schedule, facilitate, and participate in Family Team Conferences and Service Termination Conferences for families on the assigned caseload.
* Participate in individual and group supervision, team meetings, case consultations, professional development activities, trainings and workshops.
* Attend all required SWF, ACS, and external trainings.
* Maintain flexibility in work hours and location, including working remotely, in the field, within the community, or at various CA offices to effectively meet family needs and maximize time and resources.
* Perform other related duties and responsibilities as assigned.

Minimum Qualifications:

* Master's degree in social work (MSW) or a closely related field required.
* Education, training, or professional experience in trauma-informed care preferred.
* Experience working with, or knowledge of, NYC Administration for Children's Services (ACS) and child welfare-involved families preferred.

Key Competencies:

* Knowledge of New York City's domestic violence service system and familiarity with public benefits, child welfare, legal, housing, health, and community-based service systems.
* Experience providing services to individuals and families affected by Intimate Partner Violence (IPV) strongly preferred.
* Strong engagement, assessment, advocacy, and case management skills.
* Demonstrated ability to work effectively with children, adolescents, parents, and families experiencing complex or challenging circumstances.
* Commitment to a strengths-based, trauma-informed, culturally responsive and family-centered approach to service delivery.
* Strong understanding of safety planning, crisis intervention and family stabilization practices.
* Excellent verbal, written, interpersonal, and organizational communication skills.
* Ability to establish trusting relationships with clients while maintaining appropriate professional boundaries.
* Ability to work collaboratively with ACS, community-based organizations, legal professionals and other service providers.
* Strong documentation, time-management, problem-solving, and organizational skills.
* Ability to manage a diverse caseload, prioritize competing responsibilities and work independently while actively participating in supervision and team collaboration.
