Nursing at Lakewood's Care Center

There's something meaningful about caring for people you get to know-their routines, their stories, and the small changes that tell you they need extra attention.

At Lakewood's 87-bed, Medicare-certified skilled long-term care facility, you'll build relationships with residents and families while putting your nursing knowledge into practice. Our Care Center provides care across long-term care, a specialized dementia unit, and a transitional care unit, offering team members the opportunity to work with residents at various stages of their healthcare journey. Your work will help residents maintain their abilities, receive personalized care, and feel respected in the place they call home.

With day, evening, and overnight shifts available, you can explore a schedule that supports your life outside work while building your nursing career.

Whether you're starting as a Licensed Practical Nurse (LPN) or bringing years of experience, you'll work closely with Registered Nurses (RNs) and care team members to strengthen your skills in assessments, medications, treatments, and care coordination. Along the way, you'll get to know residents and see the difference your care makes.

Your Role in Resident Care



  • Provide personalized nursing care according to residents' care plans.
  • Complete focused assessments within your scope of practice, recognize changes in condition, and consult the RN when needed.
  • Administer and document medications and treatments safely and accurately.
  • Receive, transcribe, and carry out provider orders as directed by the RN.
  • Maintain timely daily charting and communicate resident needs to the care team.
  • Participate in care conferences and share observations that support resident care.
  • Coordinate assignments with the RN, assist with staff coverage when necessary, and follow up on assigned care.
  • Help team members understand nursing procedures and policies, including assisting with new employee orientation.
  • Respond thoughtfully to questions from residents and families or connect them with the appropriate person.
  • Follow infection prevention and safety practices and respond appropriately to emergencies.


Position Type

Flexible scheduling that balances life's personal commitments with resident care needs: such as 4, 8, or 12-hour shift lengths offered on all shifts, weekend-only positions, before/after school shifts, or fixed workdays each week. Rotating weekends and holidays may be required.

Available shifts may include:



  • 6:00AM-10:00AM
  • 6:00AM-2:30PM
  • 6:00AM-6:30PM
  • 2:00PM-10:30PM
  • 4:00PM-9:00PM
  • 5:45PM-6:15AM
  • and other flexible options to fit your needs!


Additional compensation is available through:



  • Competitive shift differential (PM/NOC), shift pick-up, overtime

Qualifications

Qualifications



  • Graduation from an accredited nursing school.
  • Current Minnesota LPN licensure.
  • BLS Healthcare Provider certification.
  • Completion of continuing education required to maintain LPN licensure
