Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care in skilled nursing facilities throughout Lane County. As the primary Advanced Practice Clinician for your patient panel, you'll have the autonomy to make clinical decisions, manage your schedule, and build long-term patient relationships, all within a value-based care model that prioritizes quality outcomes over productivity metrics.
