Advanced Practice Clinician (NP/PA) - Senior Community Care
Optum | Lane County, OR | Full-Time | Field-Based
Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to medically complex patients residing in skilled nursing facilities throughout Lane County. This role offers the opportunity to manage some of Optum's highest-risk Medicare members, including patients with multiple chronic conditions, advanced disease states, functional limitations, and complex treatment needs.

Working within a value-based care model, you will focus on delivering high-quality, patient-centered care while partnering with an interdisciplinary team to improve outcomes, reduce unnecessary utilization, and enhance quality of life.

Position Highlights & Primary Responsibilities
  • Conduct comprehensive assessments for medically complex patients
  • Manage acute and chronic conditions, including diagnostics, testing, and treatment interventions
  • Provide care for members during short-term sub-acute rehabilitation episodes
  • Develop and oversee complex, individualized treatment plans
  • Lead advanced illness, goals-of-care, and advance care planning discussions
  • Address medical, behavioral, functional, and social determinants of health
  • Collaborate with primary care providers, facility staff, caregivers, and interdisciplinary team members
  • Educate patients and caregivers on treatment plans and disease management
  • Ensure accurate and timely clinical documentation

Required Qualifications
Nurse Practitioners
  • Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
  • Active and unrestricted Nurse Practitioner license in the state of residence, or ability to obtain by start date
  • Board certification through AANP or ANCC in one of the following:
    • Family Nurse Practitioner
    • Adult Nurse Practitioner
    • Gerontology Nurse Practitioner
    • Adult-Gerontology Acute Care Nurse Practitioner
    • Adult-Gerontology Primary Care Nurse Practitioner
Physician Assistants
  • Graduate of an accredited Physician Assistant program
  • Active and unrestricted Physician Assistant license in the state of residence, or ability to obtain by start date
  • Board certification through the National Commission on Certification of Physician Assistants (NCCPA)
Additional Requirements
  • Current, active DEA licensure/prescriptive authority, or ability to obtain post-hire in accordance with state regulations
  • Ability to gain a collaborative practice agreement, if required by state law
  • Valid driver's license and reliable transportation
  • Ability to meet the physical requirements of the position, including transporting a 30-pound bag and navigating various facility environments
Preferred Qualifications
  • 1+ years of clinical experience, preferably in a long-term care setting
  • Experience caring for patients with complex behavioral, social, and functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficiency with electronic medical records and clinical documentation

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
