ooking for a role that offers both meaningful patient care and schedule flexibility? Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to medically complex patients in skilled nursing facilities throughout Lane County.
As the primary Advanced Practice Clinician for your patient panel, you'll have the flexibility to manage your own daily schedule while delivering high-quality, value-based care. With no productivity quotas, RVU requirements, call responsibilities, weekends, or holidays, you can focus on your patients while maintaining a healthy work-life balance.

Position Highlights & Primary Responsibilities
  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions, diagnostics, and treatment plans
  • Provide care during short-term sub-acute rehabilitation episodes
  • Lead goals-of-care, advance care planning, and advanced illness discussions
  • Collaborate with PCPs, caregivers, facility teams, and interdisciplinary partners
  • Educate patients and families on treatment plans and disease management
  • Maintain accurate and timely clinical documentation

Required Qualifications
Nurse Practitioners
  • Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
  • Active and unrestricted Nurse Practitioner license in the state of residence, or ability to obtain by start date
  • Board certification through AANP or ANCC in an approved NP specialty
Physician Assistants
  • Graduate of an accredited Physician Assistant program
  • Active and unrestricted Physician Assistant license in the state of residence, or ability to obtain by start date
  • Board certification through the National Commission on Certification of Physician Assistants (NCCPA)
Additional Requirements
  • Current, active DEA licensure/prescriptive authority, or ability to obtain post-hire in accordance with state regulations
  • Ability to gain a collaborative practice agreement if required by state law
  • Valid driver's license and reliable transportation
  • Ability to meet the physical requirements of a field-based position
Preferred Qualifications
  • 1+ years of clinical experience, preferably in a long-term care setting
  • Experience caring for patients with complex behavioral, social, and functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficiency with electronic medical records and clinical documentation

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
