Advanced Practice Clinician (NP/PA) - Senior Community Care
Optum | Washington & Multnomah Counties, OR | Full-Time | Field-Based
Join Optum's Senior Community Care team and provide longitudinal care to medically complex patients in skilled nursing facilities throughout Washington and Multnomah Counties. As the primary Advanced Practice Clinician for your patient panel, you'll practice with a high level of autonomy while supported by a collaborative interdisciplinary team.

Position Highlights & Primary Responsibilities
  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions
  • Develop and oversee complex treatment plans
  • Provide care during sub-acute rehabilitation episodes
  • Lead goals-of-care and advance care planning discussions
  • Collaborate with PCPs, facility teams, and caregivers

Care for high-risk Medicare patients with multiple comorbidities, advanced illnesses, and complex medical, behavioral, and social needs. This role is ideal for clinicians who enjoy critical thinking, independent decision-making, and ownership of patient outcomes while maintaining strong work-life balance.

Required Qualifications
Nurse Practitioners
  • MSN or DNP from an accredited program
  • Active NP license (or ability to obtain)
  • AANP or ANCC certification in an approved NP specialty
Physician Assistants
  • Graduate of an accredited PA program
  • Active PA license (or ability to obtain)
  • NCCPA certification
Additional Requirements
  • DEA licensure/prescriptive authority (or ability to obtain)
  • Ability to obtain a collaborative practice agreement, if applicable
  • Valid driver's license and reliable transportation
  • Ability to meet physical requirements of a field-based role
Preferred Qualifications
  • 1+ years of clinical experience, preferably in long-term care
  • Experience caring for patients with complex medical, behavioral, social, or functional needs
  • Knowledge of geriatrics, chronic illness, advanced illness, and end-of-life care
  • EMR and clinical documentation proficiency

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
