Short Description A Supervisor Medical Care Coordinator is responsible for the overall management and coordination of care coordinators within a healthcare organization. This role involves overseeing the administration, supervision, and monitoring of program services, ensuring that care coordinators meet the required training and maintain the quality of service provided. The supervisor also manages funding allocation, assigns and oversees care coordinators' caseloads, and ensures a respectful and supportive environment for both individuals and coworkers. Provides supervision to navigator specialist, peers, case managers and other care coordination staff as assigned. Completes bio-psychosocial needs assessment on all patients. Completes, monitors and updates service care plan. Maintains regular contact with case managed patients via phone, teladoc and/or face to face. Meets with scheduled patient to identify immediate/urgent Collaborate with community agencies while maintaining a strong understanding of community based resources and form appropriate relationships with community providers in order to facilitate appropriate referrals. Serve as a patient advocate, ensuring access to quality medical and behavioral healthcare. Experience Required 3 years experience in field. Must have experience directly managing and coaching others in related field Education Requirements High School Diploma or Equivalent required. Actively pursues a Bachelor's degree in a field of related to case management preferred. License/Certification Requirements Certified Peer Recovery Specialist preferred Hourly Rate Min $34
Hourly Rate Max $56
