DESCRIPTION

Care Navigation Hub LVN will support inbound and outbound communication from patients to assist with scheduling requests, providing information on medical services and expectations of care, offer support in understanding treatment plans, coordination of care, providing guidance to appropriate resources, and ensuring seamless communication between patients and healthcare providers.

MINIMUM QUALIFICATIONS
  • Licensed Vocational Nurse (LVN) issued by the state of California.

  • BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.

  • A minimum of two (2) years of LVN experience.

  • Demonstrated knowledge of universal precautions and proper use of body mechanics.

  • Strong written and verbal communication skills.

PREFERRED QUALIFICATIONS
  • Bilingual: English/Spanish.
  • ACLS and/or ART.
  • Ambulatory Care experience.
  • Experience with Urology nursing procedures such as catheter changes and bladder instillations.
  • Experience with sterile processing of instruments.
  • Experience with urology procedures, including prostate biopsies and cystoscopies.
  • Experience with EPIC/electronic medical record.
SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.

Pay Transparency Act

Annual Full Pay Range: $69,322 - $86,193 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $33.20 - $41.28

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
