This is an excellent opportunity for an Centralized Follow-up Specialist (32 - 40 hrs. /wk., benefits eligible) at our Jackson Street Administration Office in Milwaukee, Wisconsin.
PURPOSE of POSITION:
Under the direction of the CFU Manager and Clinical Informatics Manager, the CFU Specialist will be responsible for providing medical follow-up/education to patients as well as assisting licensed staff with CFU processes.
QUALIFICATION REQUIREMENTS:
Must be committed to providing excellent customer service and support the vision and values of Planned Parenthood of WI. Commitment to diversity, equity, inclusion, and belonging. Demonstrated abilities and experiences with working with diverse populations. Ability to carry out multiple assignments and follow through with complex assignments. Proven skills in organization, problem solving, interpersonal, written and verbal communication. Experience using MS Office 365 and Microsoft Office Products. Ability to work independently and as a team member. Ability to handle sensitive information in a confidential manner.
EDUCATION and/or EXPERIENCE:
High school diploma or equivalent. Two years of experience in a healthcaresetting, such as front desk, medical assistant, health unit coordinator and/or scheduling patientsrequired. Experience in family planning setting strongly preferred. Experience using electronic health records systems or NextGen preferred.

Physical demands of the position necessitate hearing, talking, handling, and grasping regularly. May require lifting up to 30 pounds. Specific vision conditions include close vision reading. Requires the ability to operate a computer, telephone, fax, copy machine and adding machine or calculator.
WORKING CONDITIONS:
The position is generally performed in a medical office environment but involves frequent interruptions. The noise level is generally low to quiet. The work environment characteristics are representative of those an employee encounters while performing the essential functions of this position. Reasonable accommodations, however, may be made to enable individuals with disabilities to perform said essential functions. This position can also be performed off site in accordance with the Telecommuting Policy.
Work environment, when performed from a health center, may involve exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. Will require OSHA training.
Occasional evening and/or weekend hours may be necessary.

ESSENTIAL DUTIES and RESPONSIBILITIES:
  • Provide medically accurate education for all laboratory & medical results as assigned in the CFU and accurately document in the medical record/plan.
  • Accept and manage tasks, make second and third contact attempts within specified time frame for all centralized follow-up assigned along with the necessary information sheets.
  • Run daily reportsNAD (Next Action Date) from electronic health records (EHR) to determinethe next action steps in follow-up. Which include the following:
A. CT/GC/TV/ Lab Errors
B.HPV- Pilot
C.Surge GC culture
  • Maintain next action date tickler system in the electronic health system (EHR).
  • Make appropriate appointments in PPWI's practice management system for patients at all health centers for specific treatment and follow-up of abnormal lab results, in-house referrals, and colposcopy appointments.
  • Answer phones, check CFU inbox voicemails throughout the day, return calls or task to appropriate CFU group for management. Documents client information for all calls in EHR.
  • Meet metrics for volume/time for reminder letters and contact attempts.
  • Enter necessary STI information into the State's WEDSS system within the necessary timeframes, which include CT/GC/ syphilis/ PID and GC cultures.
  • Scan all lab results as assigned into the electronic health record system.
  • Manage all incoming and outgoing medical records requests sent to Jackson St and through eFax.
  • Manage all return mail sent to Jackson St. document/ add alertsin EHR.
  • Management of all incoming medical records currently filed under Test Patient, Hold.
  • Maintain ongoing and appropriate communication with all staff inthe CFU & CFU manager as well as the patient services team.
  • Assist with quality assurance activities as appropriate.
  • Perform clerical and/or administrative duties as assigned.
  • Complete other duties as assigned.
