What we offer:

*Salary: $18.00 - $19.00

*PTO is offered for part-time employees.

Job Functions:

According to the mission and vision of the organization, supports the development and execution of strategies that impact positive health outcomes, lower health costs, and community engagement. This position is responsible for establishing and cultivating community events and health programming to families in need. Implement and maintain innovative health and wellness programming designed to encourage healthy lifestyles, increase family health education, and improve family health outcomes. Coordinates and oversees weekly Health and Wellness activities. Acts as a liaison with staff, community partners, and volunteers. Strategize, coordinate, and communicate with program leadership, volunteer services, food services, staff, and other internal departments who facilitate client health programming. This position offers a flexible work schedule with a minimum of 3 days of physical reporting to the office. This role may also require additional days of reporting, weekend support, and duties that exceed normal workday hours. Works closely with HR on Health and Wellness communications. Work closely with HR under the guidance and leadership of the Senior Health & Wellness Navigator.

Essential Responsibilities:

  1. Work with clients to develop and maintain Health and Wellness goals. Create objectives for their programs and track progress. Engage and support families as they try to increase their health and wellness outcomes.
  2. Assess nutritional needs of clients and coordinate with kitchen, outreach, and in-kind departments to ensure nutritional needs are met. Education and support to clients on nutritional elements. Work with kitchen staff to ensure client dietary needs are accounted for.
  3. Represent Metropolitan Ministries at strategic and organization supported community events, network meetings and pursue partnerships that will enhance/support services and community involvement as determined by organization strategy. Provide tours of the campus facilities.
  4. Meet personal goals established in individual development plan, KPI's, and based upon annual evaluations and through regular supervision. Actively contribute to the achievement of the team and organizational targets/goals. Supports the team in other tasks as assigned. Adheres to all policies of Metropolitan Ministries.
  5. Work with other departments to improve processes, meet growing needs, and involve supervisor as needed when there are barriers to reaching given competing demands that prevent workable solutions.
  6. Maintain weekly tracking of client health progress and assist clients in preparation of medical procedures (pregnancy, surgery, etc.) and making accommodations within their living quarters for recovery.
  7. Maintain real time notations related to Health and Wellness into HMIS database.
  8. Facilitate Health and Wellness assessments of all families participating in residential programming. Provides food bins and YMCA memberships to clients when available.
  9. Reports to the Seniors Health and Wellness Navigator for supervision and support.
Requirements

Education and Experience:

Degree in the Public Health, Health Education, or similar along with minimum of 2 years' experience providing health and wellness services for at risk population. Must understand inter-relationship of stress and trauma impact on health/outcomes.

Skill Requirements:

Must be able to work effectively with homeless and at-risk clients. This position requires the ability to work within a multi-disciplinary framework; should be computer literate proficiency with various computer applications preferred (including, but not limited to, Microsoft Office applications) with knowledge of Microsoft Word and Excel desirable. Must be able to communicate orally, as well as in writing, and demonstrate a high level of comfort in this performance, conducting and assisting in the conducting of meetings, etc. Strong interpersonal skills are required. This position requires a high degree of maturity, confidentiality, sensitivity to issues of faith and cross cultural and diversity issues, and the social skill to handle job-related matters in a professional and diplomatic manner.

Physical Requirements:

Must have adequate hearing and speaking ability as to take part in and provide sound oral communication and information. Must have a valid driver's license and be able to transport self and clients when necessary. Must be capable of working outdoors in sunlight and higher temperatures on occasion. Must have the physical, emotional, and spiritual stamina to handle job-related issues and stress. Ability to work out with clients on occasion to assist clients in creating healthy habits to meet their personal health/wellness goals and establish new health habits.

Other:

Must demonstrate legal authorization to work in the United States. Must be able to communicate effectively in English. Valid FL driver's license with a good driving record and insured vehicle in order to transport self and clients when necessary. Bi-lingual in Spanish a plus but not required.
