Responsibilities

  • Modifies medication dosages as determined by age and weight, renal function
  • Correctly interprets/documents orders and intervenes on problem medication orders/drug interactions, ADRs, DUEs, errors
  • Maintains accountability with drug use, including controlled drug inventory and management of hazardous waste
  • Performs aseptic preparation of sterile products
  • Participates in clinical pharmacy initiatives, patient teaching, code support, effective use of dosing protocols, tracking of clinical interventions
  • Supervises the care provided by licensed and unlicensed personnel, in accordance with the current Idaho Pharmacy Practice Act

Qualifications

Job Requirements:

  • Graduate of an accredited School of Pharmacy.
  • Maintains current pharmacist and controlled substance license in Idaho.
  • Possess computer skills for order entry, word processing, clinical interaction.
  • Experience with drug use evaluations/monitoring, clinical interventions. Effective communication skills with allied health care practitioners.
  • Experience in sterile products preparation, including TPN, Chemotherapy.
