We are seeking a Patient Navigation Coordinator to:



  • Assess patients' barriers to care and make appropriate referrals to remove barriers to effective breast health care services.
  • Advocate for and support patients and caregivers, collaboratively through our team and support services, as we strive to provide equitable care and outcomes for our patients.
  • Recruit and match patients for the Breast Cancer Peer Support Advocacy Program.
  • Promote the Breast Center program through community outreach activities with local groups, organizations and institutions, that are stakeholders in the improvement of cancer health disparities areas.

Education:

Minimum - Bachelor's Degree in social work; counseling; nursing, health education or related field

Preferred - Master's Degree in social work; counseling; nursing, health education or related field

Work Experience:

Minimum - One (1) year of experience working with underserved patient populations

Preferred - RN, LPN or Social Worker with oncology experience
