Position Overview: As a PRN LPN, you will deliver high-quality nursing care in the homes of our patients. Your primary responsibility will be to ensure the well-being of our patients while offering flexible and comprehensive care. This position offers the flexibility to work as needed, making it ideal for nurses seeking a balance between work and other commitments.

Key Responsibilities:

Provide exceptional nursing care to patients in their homes, adhering to individualized care plans.

Administer medications, perform wound care, and assist with other clinical interventions as needed.
Collaborate with the healthcare team to update and maintain accurate patient records.
Educate patients and their families on managing their health and wellness.
Adapt to various home care environments and patient needs, demonstrating flexibility and responsiveness.

Qualifications:

Current Licensed Practical Nurse (LPN) license in the state of Tennessee.
Minimum of 1 year of nursing experience, preferably in home health or acute care settings.
Strong clinical skills and the ability to work independently.
Excellent communication and interpersonal skills.
Valid driver's license and reliable transportation.

Responsibilities

Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
