Job Description:


Position Summary: Personal Care Worker provides
personal care services to consumer(s) of IndependenceFirst.
Personal Care Worker is directly supervised by the Care Coordinator
and the consumer(s) for whom they provide personal care services.


Essential Functions of the Job




  1. Assisting consumer with getting in and out of bed.

  2. Assisting consumer with toileting/incontinent care

  3. Assisting consumer with bathing or showering

  4. Assistance with eating.

  5. Assistance with grooming to include mouth, denture, hair,
    shaving and nail care.

  6. Assistance with dressing and undressing.

  7. Care of eyeglasses and hearing aids.

  8. Assistance with mobility and ambulation including use of gait
    belt, walker or cane.

  9. Simple transfers including bed to chair or wheel chair and reverse.

  10. Skin care excluding wound care.

  11. Meal preparation and serving.

  12. Light cleaning in areas used during personal care activities.

  13. Changing of consumer's bed, washing bed linens and consumer's
    personal clothing.


Additional Responsibilities




  1. Completion in a thorough, accurate and timely manner of all
    time sheets and documentation of consumer's condition as necessary.

  2. Keep consumer's information confidential.


The above listed essential functions and qualifications are
not all-inclusive as changes and/or other responsibilities may be
assigned as necessary.

Experience and Skills:


Minimum Qualifications

Personal Care Worker must have adequate training in the
provision of personal care services and in each skill that the
personal care worker is assigned. Documentation of training is
required. Previous experience may be accepted in lieu of formal training.


Preferred Qualifications




  1. Familiar with the philosophy and concepts of independent living.

  2. Ability to understand and follow the consumer's instructions.

  3. Good communication and to relate well to others.

  4. Good past employment references.

  5. Physical strength sufficient to perform required job tasks.

  6. Demonstrate dependability and honesty.

  7. Ability to work in a one on one setting and to be able to
    follow a written plan of duties.
