Accountable for the assessment, planning, implementation, and evaluation of patients and/or members and their families according to age-specific needs. Demonstrates the ability to work with their physical, motor/sensory, cognitive, and psychosocial limitations/needs specific to the population being served. Promotes each client's ability to manage their own health through high quality education designed to empower each person to improve and maintain their maximal health while navigating the health care continuum. Conducts 1:1 consultations, lectures and may conduct cooking school (ICR only). Assist with best practices implementation. Consult with physicians and multidisciplinary team to coordinate patient and/or member care plan. Demonstrates the ability to apply current comprehensive knowledge in the principles and techniques of chronic disease management. Provides the highest quality education/case management for patients, patient and/or members and their families, referral sources and other Renown entities. Coordinates care and services as appropriate. Demonstrates fiscal responsibility for achievement of budgetary objectives within the scope of his/her responsibility which may include accounting responsibilities such as reconciling reports and invoicing. Performs community outreach, support groups, marketing and education functions as needed. This position does not provide patient care.
