We are seeking a Forensic Case Manager to join our team in Philadelphia, PA 19119. Earn $25.67/hour working full time with benefits. Position Details The Forensic Case Management is designed to serve adults living in the community and or prison system who have serious and persist mental illness. Using an individualized approach to service delivery, the Blended Case Management program strive to promote recovery, independence, and community integration. The Blended Case Manager is responsible for developing, coordinating, and implementing services for adult participants as identified in the goal-oriented service plan. The role includes providing resource coordination, recovery services, and symptom management using a strength-based model of case management to 25-30 Adults living with mental illness. The Blended Case Manager will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence and chart documentation. The ideal candidate will possess the following qualifications: * Bachelor's Degree with major coursework in social science and two years human service experience. * Must have one year of mental health experience. * Valid PA driver's license and vehicle required. * Must be knowledgeable of the criminal justice system and resources.
