Job Summary
The Prevea Oncology Navigator assists in the development and maintenance of an Oncology Navigation Program. They work collaboratively with internal and external departments to identify consistent work standards, identify community resources, and coordinate education platforms to ensure accessibility for all patient types. The navigator assists in developing and maintaining a quality and patient satisfaction metric to ensure the success of a navigation program and collaborates with patient populations and their providers to provide care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Under general administrative direction, they give patient education related to their illness and planned treatment. Works in partnership with entire healthcare team to manage transitions of care between Prevea, HSHS, and our Medical and Radiation Oncology partners.
What you will do
* Assist and support the oncology navigation program goals. -Adapt documentation platforms, define metrics by tumor site, and support onboarding of oncology navigators. -Collaborates with providers and staff to develop standardized protocols and workflows to address oncologic needs and care gaps.
* Assess barriers to care such as population issues, gaps in care, and lacking resources. -Work in collaboration with internal and external departments to meet those social determinants of health.
* Care coordination and facilitation for the seamless delivery of care between internal and external departments
* Advocate and social support for patient to ensure patient centered care and patient expectations are met. -Attend tumor board as patient advocate. -Participate in community outreach for cancer prevention and education.
* Involvement in survivorship programs and surveillance follow ups
* Telephone triage. -Communicate with patients and providers in an appropriate timeframe. -Provide patient education using nursing judgment within the scope of license. -Triage patients to the appropriate setting.
* Work within EMR to research and document care plans
Education Qualifications
* Associate's Degree in Nursing Required
* Bachelor's Degree in Nursing Preferred
Experience Qualifications
* 1-3 years Oncology Experience Preferred
Skills and Abilities
* Ability to demonstrate strong problem solving skills
* Ability to work independently and within a team
* Ability and desire to acquire knowledge in area of specialty
* Ability to form work/patient relationships with patients and peers
* Effective interpersonal skills, strong written and oral communication as well as strong listening skills
* Understanding of community resources for Cancer Care
Licenses and Certifications
* Licensed in the State of Wisconsin as a Registered Nurse Upon Hire Required
* Obtain AAON certification in the timeframe guided by AAON Required
* CPR/BLS Certification: Basic Life Support for Healthcare Providers certification from approved program or Prevea sponsored program. Upon Hire Required
