Job Summary Plans and coordinates activities related to discharge planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. Responsible for the collaboration with medical staff, patients, family and the Healthcare team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes.Monument Health offers competitive wages and benefits on qualifying positions. Additional Requirements Required: Case Management Education - Completion of a nursing education program that is approved by a board of nursing or Bachelors in Social Works Certifications - Registered Nurse (RN) - South Dakota Board of Nursing or Certified Social Work (CSW) - South Dakota Department of Social Services Behavioral Health Education - Bachelors degree in Human Services Preferred: Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
