Applicants must have an MD or DO degree or foreign equivalent and be eligible to obtain state medical license. Successful Candidate must have completed a Family Medicine, Internal Medicine, or Med-Peds, Emergency Medicine or Physical Medicine and Rehabilitation residency program, along with a Sports Medicine fellowship. Successful Candidate must be board certified/eligible in Family Medicine, Internal Medicine, Med-Peds, Emergency Medicine or Physical Medicine and Rehabilitation. All qualifications must be met at the time of appointment."
