Work Schedule:

100% FTE, days. This position is a hybrid of in person and and remote work. When in person, you will work at 5618 Odana Rd.

Pay:



  • This position may be eligible for a $3000 sign-on bonus.

Working as an independent health care provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients while working collaboratively within a multidisciplinary health team.

We are seeking a Behavioral Health Clinician to:



  • Collaborate with other medical professionals in a fast-paced environment.
  • Provide brief evidence-based interventions.
  • Provide short, but intense follow-up.
  • Connect patients to resources.
  • Use outcome-based models to track patient improvement

Qualifications

  • Master's Degree in social work, psychology, or counseling Required

Work Experience

  • 2 years of experience in a health care environment in a patient/client contact role Required
  • 1 year of experience working with behavioral health issues in a primary care setting Preferred

Licenses & Certifications

  • Relevant license and/or certification or license/certification eligible as a Mental Health provider as required in the State of Wisconsin (i.e., Psychologist, Licensed Professional Counselor-LPC, Licensed Clinical Social Worker-LCSW, Licensed Marriage and Family Therapist-LMFT) Required
