• Provides clinical expertise in the nursing management of the patients, documentation, and care planning
• Processes start of care workflow once start of care visit has been completed
• Participates in IDG
• Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary
• Maintains orders in HCHB
• Manages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completed
• Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned
• Upholds effective communication with referral sources to ensure smooth transition of admissions

• Associate degree in Nursing with current license
• Three years practice as a Registered Nurse
• Previous Hospice experience preferred
• Knowledge of the Hospice Philosophy of Care
• Ability to use Microsoft Word, Excel, Outlook, and other software programs
