The Division of Geriatric Medicine and Palliative Care (a nationally recognized division-- ranked #2 in Geriatrics in US News & World Report) at the NYU Grossman School of Medicine, an affiliate of NYC Health Hospitals/Gouverneur, is seeking a board-certified Geriatric Medicine physician at the level of Assistant Professor to provide comprehensive medical management for nursing facility residents, ensuring adherence to CMS and NYS DOH requirements. The ideal candidate will participate in interdisciplinary meetings, respond to resident inquiries, and contribute to quality assurance initiatives.

Clinical Role and Responsibilities:



  • Provide medical management for geriatric and non-geriatric nursing facility residents including primary care, management of acute illness, and transition of care;
  • Coordinate behavioral health services for individuals with maladaptive behaviors;
  • Carry a patient case load of 100-120 residents;
  • Participate in cross coverage for absences of other team members;
  • Participate in swing shift coverage onsite, including weekends and off-site telephone coverage on a rotational basis;
  • Perform medical assessment for new admissions and readmissions and write orders for required medications, treatments, diagnostic studies and other necessary care and treatment;
  • Perform required physician visits based on resident need, facility policies and regulatory requirements;
  • Write timely orders and progress notes;
  • Initiate referrals for consultant and specialty services as required and write orders for services needed;
  • Write orders for rehabilitation services;
  • Participate in advance directives planning with the resident and representatives and write related orders;
  • Facilitate the emergency discharge of residents to acute hospitals as well as planned discharge for medical treatment;
  • Participate in interdisciplinary rounding and meetings;
  • Respond timely to calls from residents and their representatives to discuss the resident's medical care;
  • Provide medical leadership for team coordinated Quality Assurance Performance Improvement initiatives


  • Current New York Medical License
  • Board certification in Geriatrics
  • Strong communication, teamwork, and leadership skills preferred
  • Clinical and administrative experience preferred
