SUMMARY
The Social Worker I provides direct psychosocial care and discharge planning in collaboration with patients, families, case managers, physicians, and the interdisciplinary team. Responsibilities include psychosocial assessment, goal setting, behavioral intervention, and coordination of care to optimize patient outcomes. Services may include addressing psychosocial barriers to discharge, coordinating post-acute care, engaging and educating patients/families, and implementing clinical interventions to address immediate needs. Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
