SUMMARY

The Social Worker I provides direct psychosocial care and discharge planning in collaboration with patients, families, case managers, physicians, and the interdisciplinary team. Responsibilities include psychosocial assessment, goal setting, behavioral intervention, and coordination of care to optimize patient outcomes. Services may include addressing psychosocial barriers to discharge, coordinating post-acute care, engaging and educating patients/families, and implementing clinical interventions to address immediate needs.



ESSENTIAL FUNCTIONS


  • Conducts screenings and psychosocial assessments; develops and implements care plans.



  • Provides direct psychosocial support, crisis intervention, and discharge planning for assigned patients.



  • Collaborates with interdisciplinary teams to ensure care coordination and effective transition planning.



  • Maintains documentation in compliance with departmental and regulatory standards.



  • Identifies and utilizes community and regional resources to support patient needs.



  • Facilitates family meetings and participates in interdisciplinary rounds.



  • Assists with legal guardianship, psychiatric referrals, abuse/neglect assessments, and end-of-life issues.



  • Participates in quality improvement initiatives and provides coverage as needed.



  • May participate in committees, home visits, and on-call rotations.



  • Engages in required supervision per COMAR regulations.



  • Performs other related duties as assigned.





QUALIFICATIONS

Education:




  • Master's degree in Social Work (MSW) required




Licensure:




  • Licensed Master Social Worker (LMSW) - State of Maryland




Experience:




  • Up to one year of related experience preferred (field placement may count toward experience)




Knowledge, Skills, and Abilities:




  • Strong interpersonal and communication skills with ability to collaborate in a multidisciplinary setting.



  • Knowledge of community resources and care coordination.



  • Ability to assess, synthesize, and document clinical information accurately.



  • Proficient in electronic health records and general office technology.



  • Excellent time management and organizational skills.




Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
