Medical Social Worker (LCSW-C)
Job Summary: The Medical Social Worker II provides comprehensive psychosocial care to patients from admission through discharge. This role collaborates with patients, families, interdisciplinary teams, and external providers to ensure safe, timely, and coordinated transitions of care. The Social Worker serves as a clinical resource and problem-solver to improve patient outcomes and support cost-effective care delivery. Key Responsibilities:
Assess patients and develop individualized plans of care.
Coordinate discharge planning and continuity of care.
Address complex social, emotional, and financial needs affecting health outcomes.
Provide crisis intervention, counseling, and referrals to community resources.
Focus on high-risk populations (e.g., readmissions, advanced age, homelessness, psychiatric or terminal illness).
Maintain accurate, timely documentation and communication across care teams.
Qualifications:
Master's degree in Social Work (MSW) required.
Maryland LCSW-C licensure required.
Experience in an acute care or medical setting preferred.
Proficiency with Microsoft Office and electronic health records.
Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
