UC San Diego Health is seeking a dynamic and Emergency Department (ED) experienced Registered Nurse (RN) to Manage our innovative ED to Home program. This position is housed within UC San Diego Health Population Health Services and reports to Assistant Director of UCSD at Home. This role oversees an RN CM, Medical Assistant Supervisor and a team of Medical Assistants who follow patients who can transition out of the ED sooner if they meet criteria and agree to enroll in our ED to Home Program. The program functions as a virtual clinic, delivering Tele-Video Visits (TVV) to patient in their homes to promote safe transitions and tight medical management for up to 5 days post ED discharge. In some instances, the ED to Home program may enroll patients who have had a short inpatient stay but need tighter medical management to ensure unnecessary, avoidable ED returns. This person works very closely with our Care Connections Hub and UCSD at Home leaders for tight handoffs and referral between programs, along with our Accent Care Home Health Agency and other community resources. ED to Home has ED MD performing TVV Sunday - Saturday. There maybe weekend needs but schedule make accommodations when this need arises. The ideal candidate is committed and passionate to helping build this innovative program with the help of leadership and the Ed to Home team.so patients can successfully stay safe at home. Pay Transparency Act Annual Full Pay Range: $132,600 - $259,800 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $63.51 - $124.43 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs.
