POSITION SUMMARY Bring compassion, clinical expertise, and collaborative energy to a high-impact role as a Clinical Social Worker. In this pivotal position you will partner with patients, families, and the health care team to identify and address the psychosocial factors that influence medical outcomes and discharge planning. Using a broad repertoire of evidence-based therapeutic modalities, you will deliver complex interventions that promote safety, resilience, and recovery-serving as a trusted advocate and integral member of the care team in accordance with JHACH policies and standards.


KEY ACCOUNTABILITIES



  • Conduct comprehensive psychosocial assessments to uncover social, emotional, and environmental factors that affect medical treatment and recovery, and tailor interventions to meet each patient's unique needs.
  • Perform suicide risk evaluation and take decisive, compassionate action when criteria are met (including Baker Act procedures and arranging appropriate psychiatric placement).
  • Coordinate referrals and collaborate closely with multidisciplinary team members; connect patients and families to community resources to streamline care and reduce psychosocial distress.
  • Deliver therapeutic interventions across the continuum of need-crisis intervention, protective services, grief and supportive counseling, problem-solving, strengths-based approaches, psychoeducation, and coaching in adaptive coping strategies.
  • Partner with the medical team to create clear, actionable plans of care and proactively identify psychosocial barriers that may delay or prevent safe discharge.
  • Maintain timely, accurate electronic documentation, data collection, and record-keeping to support continuity of care and regulatory compliance.
  • Communicate effectively with team members about patient needs and interventions; actively participate in team meetings, rounds, and huddles to drive coordinated, patient-centered care.


QUALIFICATIONS



  • Master of Social Work (MSW) from an accredited program
  • Minimum 2 years of clinical social work experience
  • Current Florida Licensed Clinical Social Worker (LCSW)
  • Exceptional listening skills and strong oral and written communication abilities

Salary Range: Minimum 30.64/hour - Maximum 49.02/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
