DESCRIPTION

The Staff II Pharmacist will be responsible for providing comprehensive direct patient care to patients across a variety of ambulatory care settings at UC San Diego Health. The pharmacist will conduct virtual and/or in-person patient visits, focusing on the safe, effective, and evidence-based management of medication therapy for patients with acute and/or chronic disease states, including those receiving high-cost, high-risk, or complex medication regimens.

Working under applicable Medical Center policies and Collaborative Practice Agreements, the pharmacist will provide medication management, patient education and counseling, therapeutic optimization, medication monitoring, and coordination of care in collaboration with interdisciplinary healthcare teams. The pharmacist will identify and resolve medication-related problems, promote medication adherence, facilitate medication access, and support transitions of care when appropriate.

The pharmacist will support standardized medication use processes and collaborate closely with the UC San Diego Health retail pharmacies, and other pharmacy and clinical services to ensure timely access to medications and continuity of care. This position will support multiple clinical service lines and patient populations based on organizational priorities and departmental needs, serving as a clinical pharmacy generalist with the flexibility to provide care across a broad range of disease states and practice areas.

MINIMUM QUALIFICATIONS
  • Doctor of Pharmacy degree (Pharm D) or equivalent training.

  • Registered Pharmacist in the State of California.

  • Meet the minimum experience qualifications for this position in 1 of 3 ways:1) possesses a PGY 2 or 2) possesses a PGY 1 plus 1 year of pharmacist experience, or 3) possesses 4 years of pharmacist experience.

PREFERRED QUALIFICATIONS
  • Specialty medication management experience as it relates to Gastroenterology, Neurology, Rheumatology, Dermatology, Pulmonology, Hepatology, Allergy and/or Endocrinology.
SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.

Pay Transparency Act

Annual Full Pay Range: $200,010 - $238,972 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $95.79 - $114.45

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
