Career growth and development Diverse and collaborative working environment Affordable and comprehensive benefits package $5000.00 sign-on bonus POSITION SUMMARY In collaboration with patients/families, case managers, physicians and the interdisciplinary team, the Social Worker I provides direct psychosocial and practical care including social work assessments, problem identification, goal setting, behavioral intervention and implementation of care plans. The Social Worker collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to: developing action plans to mitigate psycho-social barriers to discharge; complex post-acute care coordination; patient/family engagement and education; and employing clinical interventions (to address immediate needs.
